Why Clinics Say “Anti-Wrinkle Injections”
The phrase exists partly for a regulatory reason rather than a marketing one.
Botulinum toxin is a prescription-only medicine, and many jurisdictions restrict advertising prescription medicines to the public by brand name. “Anti-wrinkle injections” describes the treatment without naming a manufacturer’s product, which is why so many clinic sites and price lists use it.
It is a useful term with one flaw. It promises action on wrinkles as a category, when the treatment works on a specific mechanism. Which brings us to the categories.
Six Things People Call Wrinkles
Movement lines
Lines that appear when a muscle contracts and vanish when your face relaxes. Frown lines, forehead lines and the fan at the outer eye all start here. These respond to injections, because reducing the contraction reduces the fold.
Etched lines
The same lines, decades later, now creased into the skin itself and visible at rest. Reducing movement softens them and stops them deepening. It does not remove them, because the crease no longer depends on the muscle. These need something that works on the skin.
Volume-loss folds
Not really wrinkles at all. Where tissue beneath the skin thins or descends, the surface folds over the gap. The fold beside the nose is the classic example. Relaxing muscle does nothing here, and our dermal fillers page covers what does.
Laxity folds
Skin that has lost its elasticity and now hangs, creating folds that shift when you change position. Neither injections nor volume address these. They are a surgical question.
Textural crepiness
Fine, shallow, closely spaced lines across an area rather than a single fold, common on the cheeks and around the eyes. This comes from sun exposure and collagen loss over years, and it is a skin-quality problem rather than a muscle one.
Sleep lines
The category nobody mentions. Pressing one side of your face into a pillow for a third of your life creates compression creases, typically vertical, typically on one side more than the other, and often on the cheek where no muscle pulls that way at all. If your lines are markedly worse on the side you sleep on, this is probably a contributor, and no injection touches it. Changing sleep position does more than any treatment.
How to Tell Which Kind You Have
Three checks, and they take a minute.
Relax completely and look. Lines that disappear when your face is still are movement lines. Lines still visible at rest have set into the skin, or belong to one of the other categories.
Stretch the skin gently sideways. If a line vanishes under tension, it sits in the tissue and volume may help. If a fine pale line persists, the crease is in the skin itself.
Compare your two sides. Markedly worse on one side, particularly on the cheek, points at sleep compression rather than muscle.
Most people find a mixture, which is normal and useful information. It tells a practitioner what proportion of your concern injections can realistically reach.
Why the Wrong Category Produces Unhappy Patients
The mismatch is predictable and it happens constantly.
Somebody with etched lines has injections, sees the lines soften slightly but not disappear, and concludes the treatment failed or the dose was too small. A higher dose next time freezes the area without improving the crease. Meanwhile the thing that would have helped, working on the skin itself, never got discussed.
The same pattern runs the other way. Somebody with strong movement lines invests in skin treatments and wonders why the lines return every time they frown.
Neither is a bad treatment. Both are the right treatment aimed at the wrong target.
Where Injections Genuinely Excel
Being fair to the treatment, it does one thing better than anything else available.
It stops a movement line becoming an etched one. Skin creases along the same fold thousands of times, and eventually the crease sets. Reducing that movement while the line is still dynamic slows the process, which is why the treatment tends to suit people who start before their lines have set.
